What a notice should include
- A physical or electronic signature of the copyright owner or authorized representative.
- Identification of the copyrighted work claimed to have been infringed.
- Identification and location of the material claimed to be infringing.
- Your name, mailing address, telephone number, and email address.
- A statement of your good-faith belief that the disputed use is not authorized.
- A statement, under penalty of perjury, that the notice is accurate and that you are authorized to act for the copyright owner.
Misrepresentations in a copyright notice may create liability. Consider obtaining legal advice before submitting a notice.
